Using Transition Assessment Data
The implementation of the Transition Assessment Plan results in the team having access to additional authentic information about the youth. This information is used to make decisions about goals for adult life, transition services, connections and next steps. It is recommended that after transition assessment information and data have been gathered, the team set a time for the team to analyze and summarize the new transition assessment data and information. At this meeting, the team will Analyze, Summarize and Draw Conclusions.
Analyze
Identify the evidence obtained from each assessment
- Each team member who gathered assessment information per the plan shares their results in response to this question.
Summarize
Succinctly describe how the new data informs the student's PINS.
- Each team member provides input about how the new data has deepened the team's understanding of the youth's PINS
Draw Conclusions
Based on new understanding, document the actions that will be taken to assist in successful adult outcomes, including refining goals, services and supports, new connections and/or additional transition assessment.
- Each team member provides perspective on meaningful next steps and identifies a potential role in each step where appropriate.
