Research shows that students engaged with 3 or more agencies before graduation have greater success in post-school employment, and education. Learn how schools, County Boards of DD, Opportunities for Ohioans with Disabilities, Family and Children First Councils, mental health boards, and more agencies can help students and families navigate transition planning. OCALI's new Multi-Agency Planning (MAP) Tool will guide you through a process of creating a shared mission, identifying a root cause, and sharing leadership to help students and families build the skills needed for adult life.

This session shares examples and strategies through our individual journeys with autism throughout the transition years into adulthood. This session shows how person-centered planning can be effective in the following areas: employment, housing, communication, independent living, self advocacy, transition from school age to adult services, college, Medicaid waivers/HCBS and so much more. Participants will leave with OCALI resources and materials to use a person-centered approach in supporting the transition to adulthood.
