Gather Existing Information About the Youth
The team starts the planning process by reviewing information that already exists about a youth's skills and plans. Reviewing existing information first assists the team to know what additional transition assessment information is needed for a more comprehensive, holistic picture of the youth's PINS and plans. Gathering the existing information together as a multi-agency team also avoids duplicating what information is already available. Existing information may also be useful to agency partners for their agency-specific processes.
