Youth Profile: Where is the Youth Presently?
By discussing what is already known about the youth and their goals, it prevents spending time and resources gathering the same information. This will allow us to craft more individualized and meaningful questions to be used for our new transition assessment gathering process, detailed in Question 4 of the process. It may also allow us to think of what is missing and which team members may be best positioned to assist the youth in gathering the information through new transition assessment (both formal and informal). We will also know what additional team members the team could consider to help support the youth. - Opportunities for Ohioans with Disabilities (OOD) is an example of a new team member who may help a youth further explore careers or even a specific job.
